Hi , my provider is asking for codes for a tenosynovectomy with possible tendon repair. It seems 25270 may be the code I'm looking for? Anyone have suggestions?
 
Unfortunately, that is still too vague. Does the patient have RA, synovial cysts? Was there an injury such as a laceration or things like mallet finger, etc?
What is the exact anatomical location or diagnosis/issue with the tendon?
You would need to know what extensor tendon and if it is to be debrided in the finger/hand/wrist/forearm.
Which digit (even if in the forearm)?
The diagnosis matters when talking about tenosynovectomy, synovectomy, or repair.
If it is a "possible" repair it sounds like the plan would be leaning more towards tenosynovectomy, you may need more than one CPT if it is for an order.
There are too many finger/hand/wrist/forearm options to narrow down without specific plan info.

If this is for a planned case to put a surgery order in, what does the visit note indicate? What is the plan?
If the case was already done, what does the operative note indicate?

Thank you so much for your reply/help! The dx is M65.842 (other synovitis and tenosynovitis in the left hand). The exam note states: Left hand dorsal palpable mass measuring approximately 4x2.5 cm that is mobile and mild tenderness palpation. Pain with wrist and finger extension. Active extension against resistance AIN/PIN/UN intact, SILT C5-T1, cap refill brisk. Left extensor digitorum communis tendon to the middle finger at the wrist: tenderness to palpation..Left extensor digitorum communis tendon to the middle finger at the wrist: full composite flexion and extension of the digits. Review of the MRI of the left wrist demonstrating severe extensor tenosynovitis of the extensor digitorum tendons with some tendinosis present. The surgery discussed was a left hand tenolysis of her extensor tendons with possible tendon repair. My thoughts are 26445 with potential 25260
