I know this may be a dumb question, but please answer and I will explain. If a patient comes in for a Screening Colonoscopy due to hx of adenomatous and serrated polyps. It's been over 4 years since the last colonoscopy.
Is it correct to code for a commercial payer: CPT 45380-33
Dx: Z12.11
Z86.0101
K63.5 since polyps were found on this screening?
