I'm an experienced coder but pretty new to Gyn Surgery coding.
My surgeon started a TLH. She was unable to remove the uterus due to an adherent ureter causing high risk for injury, but she did complete a bilateral salpingectomy.
Which is more appropriate in this situation? Should I code with 1) CPT 58571 and modifier 52 to indicate Reduced Services or 2) CPT 58570 with discontinued procedure modifier 53 and CPT 58661 for the salpingectomy?
My original thought was option 2 but I'm now leaning to option 1. Does anyone have experience with this situation? Thanks for any insight you can provide.

When a procedure is discontinued, you would look to see if any part of it was completed for which a code exists. In this case, a bilateral salpingectomy, and you would only code that.
 
A third vote for 58661-50. If documentation supports extra work in first trying to removed the uterus, consider -22 as well.
